Filing an Application

The Selection Panel strives to eliminate barriers that may prevent individuals with disabilities, who are homeless, or who may be experiencing other circumstances that inhibit their full participation in the application process. Your completed application must be received or postmarked by noon September 6, 2016. There are several options for accessing and submitting your application.

To Access an Application:

  1. Access an electronic copy of the application as an online fillable PDF or a printable PDF.
  2. Obtain a printed copy of the application at Cleveland City Hall, your nearest branch of Cleveland Public Library, or print a copy directly from this website.

To Submit a Completed Application

  1. Complete and submit via the fillable PDF.
  2. Email a scanned copy of a completed application to
  3. Deliver a completed printed application, in person, to a secured drop-off box located at the security desks at entrances to Cleveland City Hall (601 Lakeside Avenue, Cleveland, OH 44114), between 8am and 4:30pm.
